Delivered in the participant’s home or community setting, our service ensures safety, comfort, and independence while helping participants achieve their individual health goals.

Personalised and Person-Centred Care
Every participant’s health needs are unique. Distinct Disability Services Australia (DDSA) nurses work closely with participants, families, and support coordinators to:
- Develop an individualised care plan that meets medical and personal needs.
- Offer education and guidance to participants and carers for ongoing health management.
- Promote independence and self-confidence in managing day-to-day health routines.
- Coordinate with allied health professionals and medical specialists when necessary.
This person-centred approach ensures that participants feel supported while maintaining autonomy over their care decisions.
